Welcome to the Teacher's Form page. If you are a faculty member of the district and you need to turn in a form, locate your form from the list below, save it to your computer, and submit it to the appropriate personnel.

Purchase Orders

(**note** Purchase Order forms include the updated sales tax of 11.125%-effective January 1, 2017)

Short PO- The most commonly used PO. This one has just a few lines for your item entry.

Long PO- For use if you have many items that need to be placed with one vendor.
Local PO- For use with a local vendor.

****IMPORTANT:  A RATIONALE MUST BE INCLUDED WITH ALL PURCHASE ORDERS!!

Inventory

Inventory Form- This is a blank copy of the inventory sheet. At the bottom of this form, you will notice 'tabs' which indicate how many pages are prepared for inventory recording. Choose the appropriate one and type in the items that belong on your inventory. Please keep this form in a safe place so that you may reference it from year to year.
